What are the GUIDE Model benefits?
No-cost wrap around support for people diagnosed with dementia and their care partners.
Each participant receives an initial visit from a nurse practitioner and monthly support from a care navigator who works with the patient’s existing healthcare team to ensure continuity in treatement.
Up to $2500 annual respite benefit that can be used for in or out-of-home respite care, based on CMS-assigned tier.
In-home safety assessment, based on CMS-assigned tier
Care coordination by care navigators
24/7 access to resources and ongoing support
Who is the GUIDE Model for?
People who are living with Alzheimers disease and ALL other related dementias.
Those who provide unpaid care for a person who is living with dementia – this could be a family member, such as a spouse or adult child: a longtime friend or neighbor.
Care partners who are: stressed out, overwhelmed, or overburdened by caregiving responsibilities or managing a challenging medical situation.
Have little to no external support or have a complicated family system.
Need assistance navigating the complexities of healthcare.
Could benefit from respite care.
Who is eligible for the GUIDE Model?
Dementia Diagnosis: i.e. Alzheimer’s disease, vascular, Lewy body, frontotemporal or alcohol-induced dementia or primary progressive aphasia. This is not a complete list – any diagnosis that falls under the dementia umbrella qualifies.
Mild Cognitive Impairment diagnosis is not eligible.
Patient must be enrolled in Medicare Parts A and B (Traditional Medicare)
Must have Medicare as their primary payer
Cannot be enrolled in Medicare4 Advantage including Special Needs Plans
Cannot reside in a skilled nursing facility
Cannot be receiving PACE or Hospice services.
What if there isn’t a formal diagnosis?
GUIDE Model providers can provide diagnostic screenings and evaluations if there is no formal diagnosis.
